Legal Question in Immigration Law in Maryland
I am trying to file an I-90 and do not have all the information needed to complete the form. How do I obtain this information; such as: City of residence where I applied for immigrant visa or adjusted status, consulate where immigrant visa was issued or USCIS office where status was adjusted, and date of admission as an immigrant.

Why don't you have this information? It's presumed that a permanent resident should know these facts about self.
You can request a copy of your immigration file under FOIA, however, it can take many months to get it.
