what form do I use to counter a motion to dismiss by the trustee?

There is no basic form - it all depends on the grounds of the motion. What I am guessing is that you tried to 'do it yourself' and didn't file the correct paperwork, or if it's a 13, you might be short payments. If you HAVE hired counsel and you have a motion to dismiss, call your attorney. If not, in order to extricate you from a potential financial train wreck, you need to hire counsel.
