What forms do I need to go from my dba to become a corperation? Where would I get such forms? How much cost would be involved?

Forming a Corporation is relatively straightforward. You can find all of the necessary forms on-line. The Michigan forms are located on the Department of Consumer and Industry Service website under the Bureau of Commercial Services, Corporation Division.
