Subpoena
Do I have to appear in court if I have only been given a subpoena
over the phone. I have not received any form at all and I am
suppose to show up next Monday?

If the court has told you to be present n a certain day.it's best that you or an attorney shows up for the hearing. If it's a criminal case and you're being charged or there is a hearing then a warrant can be issued for your arrest.
