Do I have to use a legal format to request all documents from a past creditor? Do I have to request it from the plaintiffs attorney as well? Can I just write a letter to the original creditor requesting any and all information regarding the account?

If you are in litigation and representing yourself, you should submit discovery requests to the Plaintiff's attorney. You must have a discovery order to conduct discovery in district court.

There is a format, but if you do not follow it exactly, the judge may let it slide if you are in pro per.
