Almost all courts that require a pretrial statement have a form that they supply to you. If you didn
t get it, call the civil clerk of the court and ask for it. William S. Stern

the courts that use a pretrial statement usually provide a form to fill out. They usually want your side of the case, witnesses, etc. Contact me at www.kliszlaw.com for more info or for a free phone consutation. Tim Klisz.
