commission agreement form

What form do you recommend?

Example...An individual posts an Ad selling a Bulldozer for $20k. I re-advertise & offer the seller a buyer if he commissions me the total amount above his asking price.

The "form" would be a Commission Agreeement with provisions for protection and conditions of performance.
