Legal Question in Family Law in Minnesota

I would like to ask the court to intervene regarding medical treatment for one of my children. I do not know what form to fill out for the motion. I do have joint legal custody. I also have doctors supporting my wish. The treatment in question is being stopped by her father against medical advice. I would appreciate it if you could let me know what form/s need to be filed. Thank you.

There are no forms for such a Motion. You musyt file a Motion seeking particular action on behalf the child and support that request with any relevant affidavits and exhibits. You would be wise to hire legal counsel.
