My son is 19, his daughter is almost 2, & the mom is Bi-Polar & not taking meds

My sons ex is 19, she had her son right after her 15th b-day, her mom has custody of him. She had my sons daughter, who will be 2 in July. She is pregnant by a 17 yr She is Bi-Polar, does not take meds. My son works full time and has been paying child support since he was 16. The mom is unstable mentally, what are my sons rights to get custody of his daughter.

Since your son is paying child support, I presume that parentage has already been established for his daughter.

In that case, to change custody you will need to make a motion to the court. You should contact an attorney because if you lose your case your son will be required to wait at least two years to bring another change of custody action.
