Name Change CIS Details

I am filing to change my name in New Jersey. The packet of forms includes a CIS document, which asks certain questions. ''Name of Party'' (I assume my name, as Plaintiff), ''Caption'', ''Document Type'', and details about the defendant. Also, ''Does the statute governing this case provide for payment of fees by the losing party?''. But who is the defendant and what are the caption and document type if there is no other party involved? It is just me and my name.

You are trying to do it yourself, but you seem to be limited by your lack of knowledge about handling the matter. My fee would be $1,000 to handle the matter for you. You would have to pay the $200 filing fee and pay for any other costs.
