I've been Charged with a refusal to take a breathalyzer.

That option was not given to me...

What evidence is needed by an Officer to prove I refused a breathalyzer test?

In many counties it is required that this be recorded, as normal police procedure... At the point of trial they would then have to have the officer testify, and you would have the right to provide your own evidence...
