I have basis for civil lawsuit. is there one standard form to fill out explaining details of all particulars concerning basis for case. a standard form I can forward to a list of attorneys to see how many would be interested

Despite common misconception, there is virtually no such creature as a "standard form" or "boilerplate" of any legal document, with the exception of, perhaps, a power of attorney form. Don't prepare a complaint without the assistance of an attorney. There are numerous crucial components of a complaint which are not always apparent to the naked eye such as jurisdiction, venue, etc. If you miss one or two, you can significantly jeopardize if not destroy your entire case.

Sorry, I did not fully understand your question the first time around. No, there is no standard form for presenting the details of your case to an attorney. A simple letter or memo in Microsoft Word explaining the pertinent details is probably the best format. That way, you can email it or fax it or copy and paste the text on to an attorney's website.

No, there isn't any "standard form" to fill out. You can either write out a statement of the facts and send them off to a list of attorneys, or seek to consult with attorneys by phone or in person. The only "form" would be the grammatical conventions of the language used to convey the facts of your possible case to a lawyer. You state unequivocally that you "have basis for civil lawsuit." If so, just explain why that's so to the attorney of your choice, and see if he or she agrees.
