I have basis for civil lawsuit. is there one standard form to fill out explaining details of all particulars concerning basis for case. a standard form I can forward to a list of attorneys to see how many would be interested

There's no specific form, but any attorney should want to know the basic facts. Who did what to whom (or didn't), and why...and why it was wrong.
