Legal Question in Civil Litigation in New York

Jury Duty

My daughter and I share the same name but sometimes I use a different middle initial. There has been a great deal of confusion in the household including administrative chaos for a number of reasons. Now we have found some old mail containing a Final Notice of a Jury Questionnaire requiring a personal appearance. At first I thought it might be mine but now I'm not sure. It was dated one year ago. She or I never saw it and now I don't know how to handle it. Can we just go down to the courthouse together and explain ourselves in person and sign up for Jury Duty or are we both (or one of us) in deep legal trouble? Should I or she simply fill out the questionnaire and send it in? How do I find out who it belongs to. I have another simpler one with a different (mine) initial. Please help me.

Call the commissioner of jurors for your county and explain the situation. At least in my county, they are used to people being confused.
