Can an executor of an estate prepare release & waiver forms or does it need to come from an atty?

For a simple answer, you do not need an attorney.

It is the responsibility of the Executor to prepare the documents for probate, but normally the Executor engages a lawyer to do that work.

There is no requirement that an attorney be involved in the probate process.
