If you are using a computer generated form, you generally type your responses into the appropriate area and then hit 'print'. Otherwise you print the form out and hand-write your responses into the appropriate area. This of course assumes you have access to working computer and printer.otherwise you'd just be hitting buttons all day and nothing would happen.
