Legal Question in Medical Leave in Ohio

FMLA or sick leave?

I have plenty of sick time, and need

to take care of my wife. Can the city

require me to take FMLA? I meet all

the city's requirements for extended

sick use, but now they want me to

sign this form, which on it states''if

you fail to return to work at the end

of 12 weeks, you voluntarily lose

your job''

A couple of quick questions... Which city? Are you part of a bargaining unit? Are you a member of a union? If so, which union? What is your classification? Do you know if you are a classified or unclassified civil servant?
