My job requires that you sign an authorizationto release medical information. They request extensive medical documentation such as medications prescribed,if off work due to a mental illness they request detailed notes. My doctor released general notes and the job wants more indepth notes and not just a diagnosis. If you do not sign an authorization that states release of all notes, then your disability claim is denied. My doctor says this violates HIPA. Any doctor that I have ever seen, when given these papers have voiced the same concern.

HIPPA is not violated if you authorize the release of the information. The question is whether the employer is violating other laws (e.g., disability laws and/or the Family and Medical Leave Act) by requiring you to supply this information. However, there is insufficient information in your post to make that determination. If you have taken leave because of a disability or other medical condition, you should consult with an employment law attorney in your area
