Purchase of home with a friend

I intend to purhase a home with my girfriend and need an agreement that will:

1. Allow either of us to force the sale of the home should we have a major falling-out

2.In the event of death of either party the proceeds of deceased 1/2 interest go to their children within a reasonable time period.

Also, how should the house be titled

You need an agreement that addresses all aspects of your present and future relations, including what a "major falling out" might be, and if a sale is needed, how the respective equity interests would be determined. I am in your area.
