do you have to have, a medical poa,noterized? or filed with the courts or anything. thank you

The Oregon form of the Advance Directive (I assume that is what you are referring to) does not require notarization and it is neither recorded or filed with the court. It does require 2 witnesses to sign it acknowleging the signature of the person making the Advance Directive.
You should make sure that your doctor and those you have appointed to make decisions for you if you are unable have copies of the Advance Directive. You should also give a copy to any local hospital that is part of your medical system (i.e., Kaiser, etc.)
