I would like to know what form is neccessary in order for me to copyright my clothing designs ''name''.Example,Ralph Lauren has the name ''POLO'' for his clothing line copyrighted.Also how much would this cost and how long will the process take to be complete.

It appears that what you may actually be looking to do is to trademark the name for your design collection. POLO is a trademark. There are different kinds of trademarks and different kinds of applications, so your use of the mark, the type of protection you seek, etc. will all impact the cost and length of the process.
You can get more information on federal trademarks at the USPTO website www.uspto.gov. However, I would suggest a qualified attorney assist you in the trademark process.
