First off, only corporations are incorporated. Partnerships are formed and LLC type entities are created. It's a matter of semantics, but that's what lawyers deal with.

No. The LLC designation does not have to be incorporated into a logo design. But, it must be obvious to the reasonable man that the business being operated is a fictitious person that limits the liability of the owners and operators. The easiest way to do that is to include the corporate designation (Co., Inc., LLC, LP) after the entity name.

You should have the designation appear in any agreements you have with customers, even if you have to write it in. A corporate designation is more the more closely a name appears to be the name of a natural person. Such as John L. Schartz & Assoc. would be more likely to need the designation than Master Glass Works.
