Is it legal for a public utility to request your social security number and birth date as a condition to receive service.

You asked about a public utility requesting a SSN and birth date for service.
It is legal for them to ask, but it is illegal for them to refuse service if you refuse to provide that information. The utility can use that information for a credit review. If they don't have that information they may require a deposit on services in an amount based on the service history of the serviced location.
