Legal Question in Family Law in Pennsylvania

My brother is awaiting trial in Louisiana on federal charges. In the meantime, decisions need to be made pertaining to his finances, business, etc. His wife as mild dementia. I need to send him Power of Attorney forms as soon as possible. His residence is in Pennsylvania. The Power of Attorney will be signed and notarized in Louisiana. Which state form should I use, Pennsylvania or Louisiana?

If the agent being appointed is in Pennsylvania you would use the Pennsylvania form.

Only the appointment of the agent will be in Louisiana. The ackowledgement of the Agent will be in Pennsylvania.
