You can probably get a "form" complaint from the prothonotary's office in your county. Otherwise, you will probably have to consult a lawyer. The rules for what must be included in a complaint can be found in the Pa. Rules of civil procedure, which can probably be found on lime, if you search. If you email me, i can send you a copy orf a complaint so you know what it looks like, and the information it must contain.

I suggest you hire an attorney immediately. The manner in which a complaint has to be prepared may be too complex for you to draft. A failure to file the complaint will result in a dismissal of your case. Procedural issues are very complex. It is possible that you may win again by default or you will have to attend an arbitration hearing. It is also possible that you may be entitled to make a claim under the Pennsylvania Unfair Trade Practices Act which will allow you to collect attorney fees and punitive damages. If I can be of any assistance, please let me know. Gerald Hershenson
