Legal Question in Medical Malpractice in Pennsylvania

i was a passenger in a car accident two years ago we hit a tree head compound fracture to my arm broken sternum six ribs broken i have had three surgies and before the last one the surgeon realized that i had a torn rotator cuff since the car accident and arthritis has build up very significant. The same surgeon did all of my surgeries and i just dont understand why it took him this long to treat my injury and i was just wondering if he was at fault or the hospital because i was in there for a week and my surgeon told me that that hospital wasnt doing there job apparently but i believe its the other way around i just want to know what can i do this car accident really messed up my future as a welder iam only 22yrs and i live in constant pain
