You will need to have a custody agreement or a guardianship agreement drafted and filed with your county family court. Generally, the schools will not accept papers that are not filed with the court. You could call the school and ask what they will accept. Please make an appointment with a local attorney to discuss the steps necessary to make sure an agreement is done properly.
