How to properly complete and submit an individual subpoena request in order to have legal document served to requested party?

I don't know about the county of your lawsuit, but in Dallas County, the District Clerk has the form to complete. If the person being served is in the county then constable will do it. If they are out of county, you will have to hire a private process server.
