What form can be used for my Mother who wishes to give her present house to my sister, and our original home to my brother?

I need to know a few things first:

1. Is mom competent?

2. Is mom planning to be on Medicaid within five years?

3. Is mom the only one on the title to the homes?

In most cases some form of a warranty deed. Most title companies or attorneys will do this for you for a few hundred dollars. (Don't use a DIY deed--the amount you save isn't worth it if it gets messed up.)
