Sign over deed

My ex boyfriend and I bought a home in Dec04. I want to get my name off of the mortgage and deed and anything else associated with the house. The purpose of this is for the house not to be considered an asset. He is in agreement to this. I want no money out of the deal. What would be the correct form to file? Thank you for your assistance.

I'm assuming both of you purchased the house together, and both are on the note and deed. Unless the ex refinances, you're stuck on the note. You can deed out your interest in the land, such that you won't have any ownership in the property, but you will still be liable for the payments on it if ex doesn't pay. It will also still be on your credit report as a liability.
