Name Change procedure form
Years ago I started filling out name change procedure forms for my 1 1/2 year old daughter. I then put it on hold and now, 3 years later, I would like to continue with it. However, in those 3 years, some of the information that I originally filled out has changed (daughter's age and address). Do I need to start over with new forms or can I just cross out the incorrect information and put the new info? Or, could I use ''white-out'' to cover the old info? I thought I heard somewhere that ''white-out'' is a no-no on legal forms, but I'm not sure. Thanks.

White out might be accepted. Go the Clerk of Courts office and they may be able to find someone who can help you or get new forms, or check around and get some price quoutes from some attorneys. Best wishes, Mark J. Mahoney
