termination of parental rights

i am wondering what the name of the form is that i would have to have signed in order to have the parental rights terminated for a child i am currently pregnant with. the father has agreed to sign his rights away. also if i can have it mailed to me and what the cost would be for that. thanks, tanya

Hello,

It is not possible to just mail you a form. The courts take the matter seriously. Notice has to be given, there is a hearing scheduled, and it has to be confirmed that the person signing does so voluntarily. I should not cost much to get this project done, but you need to talk to a lawyer in your area to see what your options are.
