Accident Occuring at Nursing Home

My grandmother is presently in a nursing home for rehabilitation on a broken hip/leg. On Sunday she was admitted with no complaints regarding her shoulder. On Tuesday a Occupational Therapist observed her being unable to move her right arm. An xray was done revealing a fractured shoulder. Friday she underwent surgery for shoulder replacement. The nursing home is saying that she was injured prior to coming to the nursing home. My grandmothers personal physician is stating that she did not have any other injuries besides the hip/leg. What do I do legally? Can I do anything legally?, and if I can do something where do I go?
