Motion to the Court Regarding Child Support

I need a copy of a Motion to the Court regarding child support for my two children. Both children are living with me now, one of whom was living with his father, but now lives with me until he graduates in 3 years. Father has disappeared, is homeless, don't know where he works, where he living, etc., but believe him to be in the State of Colorado. Need to ask court to assign child support and have it documented in the court files that my oldest son is living here so that I can receive child support. Just need a sample to go by.
