I want to ask some advices from you, I AM JESSICA HEMOTIA LAMBOSO,22 Years of age my mother's name is JASMIN GEMOTIA LAMBOSO.In my birth certificate my middle initial was H. and not G. from elementary to High school and college I carried this HEMOTIA middle name but my sister she carried GEMOTIA.My problem is when I will take my education board exam this incorrect middle initial I have would affect my application?...Please advice me....thank you...

Jessica you have to visit the local civil registrar where your birth was registered and you can have it corrected administratively. They have this form which is yours for the asking.
