Name deletion and name change on deed

My deceased husbands name is on my deed and I've remarried and taken a new last name. I'm trying to get a home equity loan but can't until the deed is in my new name only. What forms do I need to get his name off and my name changed.

Need more information. Did deceased husband die with a will? Was his estate taken care of? What was contents of will, or if none, family situation? All of this information is necessary to figure out the proper way to tackle this problem.

You may be able to get help from a title company, which may be able to give you advice on how to clear the title, depending on the specific facts of your situation.
