How to Reinstate a case to active docket

I just need to know how to petition the court to reinstate a case to the active docket. I felled to appear in court and the case was set to inactive. I just want to send the proper form in to reactivate the case without going through a lawyer.

Go to the clerk's office and ask for a form which can be filled out to serve as a motion to reinstate the matter to the court's docket.
