Deminished Capacity Legal Form

My grandmother has alzheimers and my Aunt has power of attorney, my grandmothers doctor told my Aunt that she needed to get a legal medical form that states that my granmother in-deed has alzheimers and is unable to make decsions, what legal medical form is this and where can I get it?

There is no simple 'form'. Doctors shouldn't try to practice law. There are boilerplate 'do not rescusitate' forms and 'living wills', but she is already past the point of competentcy to sign them. The same applies to her making a will or trust. You probably will need to obtain a conservatorship over her by filing an action in the probate court. That would give you authority and responsibility to make financial and care decisions for her. Contact me if interested in doing so.
