can an employer demand a doctor's note from an employee when they do not provide insurance.

A note can be demanded by an employer for a number of common reasons: i.e. to substantial absence, to substantiate accommodation, or to confirm fitness for performing the job. An employer has the right to request a note for one of those or similar purposes regardless of whether they provide insurance. Insurance benefits are simply irrelevant.
