Withdrawing a Guilty Plea in a Traffic Proceeding
Is there a simple form for withdrawing a plea of guilty in a case concerning no insurance? A friend entered a guilty plea to a no insurance charge because the insurance company said the policy had lapsed, she later found she was fully covered.

There is no real easy form. You have to present the court with a motion to withdraw the plea. You have to start the whole process over.
