I need help on what release/form to use
If I am filming a concert for an artist and they are happy to give permission which form do I use?
Copyright License Agreement (Videographic Work) or Copyright License Agreement (Music Work)

Both or have an attorey combine them and pick the right law, venue, etc. for you.

If you're asking a question like that, you need to hire an attorney. If it's a work-for-hire, you need to make sure that the artist indemnifies you for music use. If you're going to own the resultant program there are many other considerations you need to take into account.
