Eviction problems

Tenant has failed to pay rent. We have given written three day notice, and no open comunication w/tenant, threats during a past phone conversation to our property,(have police report)lack of consideration for property manager, refusing to leave now and at end of lease.Lease runs out in two months, should we evict, what is process, can we do this on our own?How much will this cost me? She shouts obsenities and hangs up on us and doesnt answer door. We are out of state. Help!
