my dad passed away 3 mths ago,i have POA at the bank;Now the bank froze the account and said that i need an affidavit in order for me to continue using the account.I am the only survivor,i live in fla,what form do i need?.Thank you.

Your Power of Attorney became null and void when your Dad died. It now has no legal effect. You need to probate your Father's estate. Until a Personal Representative has been appointed to your Father's estate in his probate case, no one will be able to access those bank funds. You need to hire a probate attorney as soon as possible.

A power of attorney is no good after death. You need to open an estate.
