Legal Question in Real Estate Law in Indiana

If I have a mortgage on a new house, how should the name(s) on the deed be recorded at the county recorder office? Does it have my name and the bank's name? The bank's name only? Also, should this newly mortgaged house be recorded as a Corporate Quit Claim or a General Warranty Deed? Please clarify how this all works.
