Do I have to show up to court if I am subpenaed, and what will happen if I don't go?
Do I have an option to decline a court appearence?

If you are served wih a summons you must appear in court. A summons or Subpoena is a court order. If you do not appear the attorney that issued the Subpoena can ask for a capias and have you picked up and brought to the courthouse. That means a police officer or constable can place you in handcuffs and take you to the courthouse.
You have no option to decline to go to court.
