What's the difference?

When would you use one form vs the other?

Without knowing the context, it is difficult to provide assistance. An affidavit is a statement made under oath. The oath is often administered by a notary public, who also signs and stamps the document with an official seal. A declaration (of what, facts?, a theory of the case?) is perhaps not made under oath, although I would have to know in what context it is being provided.
