Revoking a Living Trust

I wat to rvoke a living trust that I had made in California in 1983. I have lived in Nevada for 15 years.Should I use the California or Nevada forms?

It does not matter what form you use as long as it is notarized and delivered to persons or institutions which have your property in trust. You will have to take the property out of the trust and put it back in your name too.
