Good morning, What to do if your passport was lost a while back with your tourist Visa and now your husband is filing for you and that Visa number is required and you don't have it but you know the passport number and date Visa was issued to you.

You need to request a copy of your I-94 card from USCIS with Form I-102. You should do this prior to your husband filing an application for you, as USCIS may not be able to approve your application for permanent residence without proof that you entered the US with a valid visa. You can find information about the I-102 at www.uscis.gov.
The USCIS will require from you a proof that you entered the USA legally on a visa. If you lost a passport, visa, I-94 card -- it could be a problem which you have to solve before applying for adjustment of status.
Form I-102 is used to request a copy of the I-94. A US Consulate can be contacted to obtain verification of a visa, if necessary. You shall keep in mind that without all necessary documentation, the USCIS is not going to take your word that you had a visa and were inspected and admitted to USA.
