My husband has a custody agreement of every other weekend with his daughter. This upcoming weekend is his and he daughter has informed him that her mom is taking her to connecticut...without asking him. What can we do and what penalties can she face?

It's not what we can do it's what your husband can do. Technically what the child's mother has done is contempt. That said if it's the first time she's violated the order they aren't going to punish her very much.
I suspect that the children's mother gets them for Mother's day. That would be in the order if she does.
The best thing to do is have your husband talk to the child's mother to find out what she's thinking and why. Then try to work out an arrangement so that your husband can get some make up time.
