If you received medical from state do you have to pay back from back-pay, from disability?

Yes. When you are awarded SS disability, they initially process the case as SSI and repay the state directly.

By "medical" if you mean medical insurance or Medicaid, you are not required to reimburse the Commonwealth.
If you receive cash assitance, the Commonwealth has a lien on your SSI proceeds and the SSA will pay the lien directly.
